Mobility & Fall Prevention Post-Discharge

Carely caregivers provide attentive mobility support and environmental awareness — keeping pathways clear, assisting with transfers and ambulation, and observing changes in balance or strength that should be reported to the family or medical provider. This is non-medical observation and support. Any clinical concerns are communicated to the family immediately.

Post-discharge is a period of high information flow — discharge instructions, follow-up appointments, medication changes. Carely caregivers do not manage medical decisions, but they help families stay informed about what they observe day to day: appetite, energy, mood, and physical changes. This ongoing communication helps families catch problems early and coordinate with clinical providers without relying on the senior to self-report.
